  • If you are scheduled for a travel consult, you can save time by completing the Travel Medicine Questionnaire before your appointment.  You can fill out this form online , or you can print a copy of it (click here for PDF document) and bring it with you.

  • bring the medications you're currently taking
  • bring your health card, personal identification
  • bring credit, debit or cash for any non-insured services
